Icatibant (Firazyr) is a synthetic decapeptide that blocks the bradykinin B2 receptor. In hereditary angioedema, a lack of working C1 esterase inhibitor leads to excess bradykinin, which makes blood vessels leaky and causes swelling of the skin, gut and airway. Icatibant contains five unnatural amino acids that make it a potent, enzyme-resistant antagonist. The FAST trials led to its authorisation for treating acute attacks by injection under the skin.
